The intention of this form is to gather important data to determine what the potential might be for team offerings for the 2024-25 hockey season. We encourage all current Gazelle registrants and others to complete this form. The form is not binding and is simply a planning exercise. Current Gazelles families are welcome to share this form with any other hockey families who might be considering Gazelles for next season. This form can accommodate for up to two children.
